Research Goal
The long-term goal of Dr. Wang’s laboratory is to elucidate mechanisms underlying the pathogenesis of cardiometabolic-based chronic disease (CMBCD), and to optimize current diagnosis, risk stratification strategies, and treatment modalities of CMBCD.
Adipo-vascular axis in CMBCD
We focus on the casual relationship between adipose tissue and the pathogenesis of cardiovascular disease through multimodality imaging analysis, tissue proteomics, and basic research studies. We have identified a novel adipocytokine superfamily, C1q/TNF-related proteins (CTRPs), and demonstrated its critical role in atherogenesis through the adipo-vascular axis (Eur Heart J, 2016; Atherosclerosis 2016). We have been analyzing the synergistic and antagonistic effects of different CTRP members in regulating vascular injury and homeostasis, as well as their specific receptors.
Optimized management of CMBCD
We have established a large cohort (CAMPUS) comprised of patients with cardiometabolic disorders to optimize diagnostic and treatment strategy, and their clinical outcomes. We showed hyperglycemia and glycemic variability were strong predictors of adverse left ventricular remodeling and compromised myocardial functional recovery in patients with type 2 diabetes (Cardiovasc Diabetol, 2019, 2020; Int J Cardiol, 2022). On the other hand, we demonstrated that insulin resistance was a fundamental risk factor for impaired development of HFimpEF in non-diabetic patients (J Am Heart Assoc, 2022). We have been identifying novel metabolites and developing precise and high-throughput assays to aid in early risk stratification and intervention.
AI-based risk prediction of CMBCD
We are now developing AI-based algorithms to predict risk of coronary events from CTA data by integrating vulnerable plaque characteristics, CT-fractional flow reserve, and wall shear stress. This new AI-based model will profoundly improve the efficiency and accuracy of coronary risk prediction in a non-invasive way, especially in patients with only mild coronary artery stenosis. We are also characterizing myocardial and epicardial adipose tissue features, as well as strains to predicting myocardial remodeling and functional recovery.
